Unofficial Transcripts
While enrolled, students may request unofficial copies of transcripts for personal review. Students may request only one transcript per semester. There is no charge for this service.
Official Transcripts for the MTCM program
The first three official transcripts are sent to the student, the California Acupuncture Board (CAB) and the National Certification Commission for Acupuncture and Oriental Medicine (NCCAOM) free of charge.
Requesting Transcripts
Students and graduates may, at any time, request official transcripts by mailing, faxing, or emailing a signed Transcript Request Form and $5 (per transcript) to – Mail: Five Branches University, ATTN: Transcript Request, 200 7th Avenue, Santa Cruz, CA 95062 | Fax: (831) 476-8928 | Email: studentservices@fivebranches.edu. The request must contain the following information: name (current and at time of attendance), contact information (address, e-mail, etc.), last date of attendance, address where the transcript is to be sent.
Transcripts will not be released if a student has any outstanding financial obligations to the University (including fines accrued from overdue or missing Library material, parking violations and Clinic fees). When requests are made for rush delivery/special delivery services there is an added cost of $10 (per transcript) to be paid in advance by the student or alumni requesting the transcript.
The University maintains all records required by the California Reform Act at its primary administrative location for a minimum period of five years.
