Before entering the field of Chinese medicine, he earned a PhD in Applied Mathematics from the University of California, Davis, as well as degrees in mathematics and physics from the University of Maryland. BAD ZURZACH, Switzerland — Five Branches University Professor Brandt Stickley led a two-day clinical workshop exploring Chinese medical psychology, trauma, and Shen-Hammer Pulse Diagnosis during the international acupuncture and Traditional Chinese Medicine gathering held in Bad Zurzach, Switzerland.

Her workshop, “The Eyes and Chinese Medicine: Eye-System Theory and Clinical Applications,” took place August 15–16 and introduced participants to Mu Xi (目系), or the Eye-System, a clinical framework for understanding the eyes within the larger network of Chinese medicine physiology.

What Can You Expect From the Career?

A license lets you begin practicing — it doesn't guarantee a job, a full schedule, or a particular income. Here's what building the career on the other side actually involves.

Once you're licensed, you'll choose among different execution environments — and the biggest differences between them usually come down to where your patients come from, how you're paid, how much administrative support exists, and how much marketing and running-a-business responsibility falls on you.

There's no single career path

Licensed acupuncturists work in private practices, group clinics, integrative health centers, medical offices, community clinics, and educational institutions — as employees, independent contractors, or business owners. Not every graduate enters a clearly defined salaried position; some find employment in an existing clinic, others begin part time or share office space while gradually building an independent practice. Clinical ability alone often isn't enough — finding a location, attracting patients, and managing operating costs all become part of the job.

Join an established practice

Many new graduates start as an Associate Acupuncturist inside an already-running practice — paid hourly, on a fixed salary, a share of patient revenue, or some mix of the three. It's a way to build real clinical experience without carrying the full cost of opening your own clinic. Group practices, where several practitioners (sometimes across different specialties) work under one roof, can offer steadier referrals and shared administrative support — but confirm how income, scheduling, and patient ownership actually work before you join one.

Genuine mentorship — direct supervision, real case discussion, honest feedback — can teach things the classroom can't. It's worth telling that apart from a practice that uses the word “mentorship” mainly to recruit low-paid associate labor.

Start your own practice

Independent practice gives you control over services, scheduling, fees, and branding — and puts rent, insurance, recordkeeping, compliance, marketing, and finding your own patients squarely on you. A shared office, splitting space with a massage therapist, physical therapist, or other provider, is a common lower-cost way to start; confirm the lease terms, patient privacy, and liability-insurance arrangements before signing anything.

Practitioners who run their own clinics spend real time on work that isn't direct patient care — scheduling, billing, bookkeeping, marketing, supplies, records, and insurer communication. Some people value that independence; others prefer employment or a group practice where more administrative support already exists. Either way, building an independent practice takes real time — a new clinic doesn't produce a stable caseload overnight, so budget a financial cushion while you establish yourself.

Community clinics and medical groups

Community health settings serve a broader mix of patients — often including low-income, immigrant, elderly, or chronically ill populations — and can be a meaningful way to practice while working alongside primary care and other specialists. Not every Federally Qualified Health Center (FQHC) or community clinic offers acupuncture, so confirm it's an actual funded position before assuming the opportunity exists.

Larger medical groups — Kaiser Permanente, Sutter Health, Sharp HealthCare, and similar systems — sometimes place acupuncture within pain management, rehabilitation, or integrative-medicine departments. These positions are fewer in number than private-practice openings, but usually come with steadier pay and built-in collaboration with physicians, nurses, and rehab staff.

Some practitioners move toward teaching — as faculty, or as a clinical supervisor overseeing students in a teaching clinic — which usually requires a track record of clinical experience first. Research roles, such as clinical trials, case-data work, or outcomes analysis, exist at some institutions and come with their own protocol and research-ethics requirements.

Most practitioners also develop a clinical focus over time — pain management, women's health, fertility support, oncology support, sports medicine, or pediatrics, among others — usually built gradually through casework, continuing education, and mentorship, rather than declared on day one. A stated focus doesn't expand your legal scope of practice or justify claims beyond your training.

Patient trust — and income — take time to build

A new license doesn't produce a full patient schedule on day one. Patients choose a practitioner because of referrals, reputation, communication skills, location, language, and clinical focus — trust within a community and relationships with other healthcare professionals both take real time to build.

Income varies widely by location, employment type, patient volume, insurance participation, and years in practice. What Happens After Graduation?

Graduation is an important milestone, but it doesn't automatically make someone a licensed acupuncturist. Most graduates still need to pass examinations, submit an application, and meet the requirements of the state where they want to practice.

A degree and a license are different things

The school awards your degree; a state licensing board gives you legal permission to practice — finishing school makes you eligible to move into the examination and licensing stage, not licensed outright. And your state determines the path: each state decides which education it accepts, which exams are required, whether national certification is required, and what the licensed practitioner may legally do. Investigate the state where you may want to practice before choosing a school — not after graduation.

California's pathway: the CALE

California runs its own licensing pathway built around the California Acupuncture Licensing Examination (CALE), currently offered in English, Mandarin, and Korean. Applicants must first meet the California Acupuncture Board's education requirements, pass the CALE, and then complete a separate initial license application — passing the exam is not the same as holding the license. CALE uses a criterion-referenced pass standard, meaning it judges whether you've met a minimum safety bar, not how you ranked against other test-takers that day.

Here's what the California pathway actually looks like, step by step:

Confirm your eligibility pathway. Most applicants qualify through a California-approved education program. The Board also recognizes a Tutorial Program and certain foreign-education pathways — confirm which one actually applies to you before you apply.
Apply to the California Acupuncture Board. Submit your exam application and fee directly to the Board. Applying to take the CALE and receiving your license afterward are two separate steps, not one.
Submit your documents. Typically an application form, identification, official transcripts, and proof of degree or program completion. School records usually have to come directly from the school, not from you.
Board review. The Board checks your application and education for completeness. If anything's missing, you'll get a deficiency notice — keep your contact information current and respond promptly.
Receive exam eligibility. Only once the Board confirms you meet requirements can you move into scheduling. Graduating doesn't automatically get you into the exam room.
Schedule the CALE. Eligible applicants are notified how to schedule a seat. Dates, locations, and procedures follow the Board's current official Exam Handbook.
Take the exam. The CALE is currently offered in English, Mandarin, and Korean. Review accepted ID, check-in time, and prohibited items before exam day — exam security rules are strict.
Get your results. Results are released on the Board's current timeline. CALE uses a criterion-referenced pass standard — it measures whether you've met a minimum safety bar, not how you ranked against everyone else who tested that day.
Complete your license application. Passing the CALE isn't the license itself. You still need to finish the initial license application before the Board issues your California acupuncture license number and you can legally begin practicing.

Most other states: NCBAHM national certification

Many other states use exams or certification administered by the National Certification Board for Acupuncture and Herbal Medicine (NCBAHM), covering areas like Chinese medicine foundations, point location, biomedicine, and Chinese herbal medicine. Some states require full NCBAHM certification; others require only certain modules plus their own state-specific conditions.

This distinction can be confusing: NCBAHM certification verifies that you've met national education, exam, and safety standards — it does not, by itself, issue a state license. A graduate may need both national certification and approval from a state licensing board, and the exact combination depends on the state.

Confirm your eligibility pathway. Check the current NCBAHM Certification Handbook to see which pathway and exam modules apply to your education and the state where you plan to practice.
Submit your NCBAHM application. Done online. Use a name that matches your ID and your school's records exactly, or it can hold up your file.
Submit your documents. Your school typically sends official education documents directly to NCBAHM. Your eligibility review has to clear before you can schedule anything.
Get your Authorization to Test (ATT). Once your eligibility is approved, NCBAHM issues an ATT — you can't book through Pearson VUE without one.
Schedule through Pearson VUE. Choose a test center, a date, and pay the exam fee. NCBAHM exams run at Pearson VUE Professional Test Centers across North America and other regions.
Take the required module(s). Depending on your certification and state, this may include Acupuncture with Point Location, Foundations of Herbal Medicine, Chinese Herbology, and Biomedicine — confirm exactly which modules your target state actually requires.
Get your NCBAHM Board Certification. Once you've completed the required education, exams, and other conditions, NCBAHM issues Board Certification. This is a national credential — it is not, by itself, a license from any state.
Apply to your state licensing board. Finally, apply to the state where you intend to practice. The state decides whether to issue a license, and may add its own background check, state-law exam, or additional paperwork.

The exam is a real hurdle — and the application takes time

Licensing exams aren't graduation formalities — they can test years of material across diagnosis, point location, treatment planning, herbal medicine, biomedical knowledge, ethics, and patient safety. Graduates often need a separate period of focused exam prep, and a failed exam can mean added fees, more preparation, and a real delay before entering practice.

After meeting exam requirements, applicants may still need official transcripts, exam results, identification, safety certifications, and application fees before a license is actually issued — missing records or incorrect information can create delays, so expect a real gap between graduation and being legally ready to practice.
